From select&Input I took the fields.

How can I set the variables for the form and where it should go to?
What javascript must go in ?

I’m trying to create an app which remains good.
It is simply 4 pages of a website and 1 form

1: If I have a design it looks after uploading different
2: Importing a frame is possible, but the design always differs
3: Also after your excellent explanation the sendbutton for a form is
for me a mistery
