i would like to have step by step instruction to upload my app to the appstore. from distribution provisioning profile, do i need another program like iphone configuration utility to install the provisioning profile or another program for app signing or this is done by myeclipse phone gap build? after that I will need application loader,correct?
and finally add the build to iTunes connect. am I missing something?

Here are some resources:

1) you will need to use itunesconnect to setup your app in the itune appstore. Apple provides lots of info for how to do this:

2) You will need to sign your app using an appstore distribution provision profile.

3) Lastly to upload your app to the itunes appstore you will need a mac running the most recent version of the Application Loader utility. It is described in the 2nd like of #1 above. If you don’t have a mac with the utility you migth consider using an upload service which charges a small fee such as https://appstoreuploader.com/
