Is anybody here can help me with my Problem please ?

My Big Problem :
I need a Mail form with this example. Incuded Date and Time.And working fine.

My Questions :
– what is the Steps for running the Form ( i check all the steps what is here) ( i also download the sample forms-is
not working by my)
– where is the php file for config the sending email Adress ?

– is anybody here can develop my form complete so i put only the email adress for sending inside ?
I will pay also. I am happy I have after a perfekt working email form with Datum and Time inside
